Those signs belong to a cluster doctors call cardiovascular-kidney-metabolic disease, which can later include kidney damage. The study shows a link between childhood protein patterns and adult disease risk, not proof that the proteins cause it. It did not measure kidney filtering rate (eGFR) or protein in the urine, so the kidney part of that link is still indirect.
